NURSING PROCESS

Definition: Nursing process as the frame work of modern practice can be used by any professional nurse to impliment a comprehensive, quality and eidence-based nursing care to client (Nwonu 2002).

According to Yura and Walsh (1978) Nursing process is an orderly systematic manner of determining the client problem, making plan to solve them, innitiating the plan or assignning others to do so and evaluating the extent to which care given were effective in resolving the client problem (this definition was accepted by Nursing and midwifery coucil of Nigeria).

ASSESSMENT (HISTORY TAKING)

Theory of GORDON MAJORIE,S Eleven (11) functional Health Patterns that can be used and was endosed by (N&MCN) in Nursing History taking include.

1   Health perception/health management- state of health of past and present illness.

2    Nuitriion and metabolism: Nuitrition- ask question about food taken, superstitious belief, allergy, normal dietary itake etc.

3   Elimination: perspiration, vomitting, diarrhoea, urination etc.

4   Activity/exercise : wheather patien has sufficient energy to complete desire activity, exercise pattern , ability to do daily activities without getting tired, how comfortable she is when doing house work, is there breathlessness on climbing staires.
